Rising medical costs and mounting duty-of-care obligations are pushing multinational employers to treat internationally mobile employee benefits as a governance priority rather than an administrative afterthought, according to WTW’s latest Internationally Mobile Employee Benefits Design Survey, published in September 2026.

From Benefit Provision to Active Governance

Though internationally mobile employees represent a small share of most workforces, their exposures can be outsized: a single medical event, evacuation or death may trigger claims spanning multiple contracts and jurisdictions. Complicating matters, traditional expatriate assignments now coexist with commuters, project workers, business travelers and remote hires, creating eligibility gaps that a tidy policy summary can mask until a real claim falls between vendors.

A Practical Governance Framework

WTW’s framework calls for four governance steps: maintaining an accurate inventory of mobile employees and assignment types, mapping clear responsibilities among HR, brokers, insurers and assistance providers, separating financing decisions from service quality, and tracking management information beyond annual renewal totals, including claims severity, evacuation frequency and provider response times.

Employers are advised to launch cross-functional reviews spanning mobility, finance, legal and procurement teams to identify uninsured or duplicated coverage and stress-test major-claim scenarios. Vendor oversight should extend beyond price and network size to claims governance, data security and business continuity.

As medical inflation persists and workforce mobility diversifies, brokers and insurers capable of delivering comparable data and decision support – rather than another benefits catalogue – will likely gain competitive advantage in this increasingly scrutinized corner of global benefits.
